Novak Djokovic has revealed some shocking long-term health issues after the tennis legend was defeated in the second round amd was captured struggling physically on the tennis court.

The Serbian suffered a surprising defeat at the 2026 Cincinnati Open after struggling with a recurring health condition aggravated by the heat and humidity. The 39-year-old, a three-time champion in Ohio, lost 2-6, 6-4, 6-4 to Argentina’s Thiago Agustin Tirante.

Djokovic started strongly but faded as the match progressed, requiring medical attention and appearing to vomit on court. The player was captured struggling as the video went viral among fans, raising concerns.

Speaking afterwards, the 24-time Grand Slam winner congratulated his opponent but was frank about his physical difficulties. He revealed to Sport Klub:

“It was not an enjoyable match for me, for sure. The way I felt. Not the first and I guess not the last time, but it is what it is.”

The tennis icon explained that the problem stemmed from an underlying health condition that has troubled him for several years, particularly in hot and humid conditions. Temperatures around 28°C and humidity near 80% proved especially challenging.

Looking ahead, the former World No. 1 expressed doubt about returning to Cincinnati. Djokovic stated that he hopes to return to Cincinnati next year but, “at this moment it seems unlikely” that he will.

Meanwhile, the result marks a difficult return to the tour for Djokovic after his Wimbledon semi-final loss in July. For Tirante, ranked around world No. 50, it ranks as the biggest win of his career. He will face Martin Landaluce of Spain in the Round of 32.
